On behalf of the CUPE Local 4207 Benefits Committee, Canadian Benefits invites you to maintain your coverage at the lower subsidized rate in the Health and Dental Benefit Plan for the Spring/Summer Semester even if you do not have a contract.
We are pleased to advise you of the following plan parameters:
YOU CAN MAINTAIN YOUR COVERAGE:
Because you Enrolled for the Winter Semester, you now qualify for Employer subsidized rates for Single, Couple or Family coverage and can remain covered for the Spring/Summer semester (June 1, 2019 – September 30, 2019). The premiums for the Spring/Summer Semester will be the same as for the Winter Semester; payment will be deducted monthly from your bank account or you can submit monthly post dated cheques. Monthly rates are:
Single coverage – $30
Couple coverage – $150
Family coverage – $325
Direct premium withdrawals: Simply forward your banking information or void cheque along with the completed Enrollment Form and we will withdraw the semester’s premium directly from your bank account. Previously, premiums were withdrawn for the entire semester. In response to Plan Members requests, premiums will now be withdrawn on a monthly basis on the last Friday of each month of the Semester.
If you have previously submitted your banking information and wish to have premiums deducted again from your bank account, please advise us in your email.
Cheque: Payment can also be made by monthly post dated cheques or bank draft, payable to “CUPE Local 4207”.
To qualify for the extension of your coverage with Employer’s subsidized rate, please advise us by email by Tuesday June 11, 2019.
When you enroll, you will receive a confirmation email from us once your coverage has been activated.
